This website is not intended for the transmission of sensitive or protected health information. While reasonable safeguards are in place, online communication may not be fully secure.
Formal patient communications, medical records, and protected health information are handled through secure, HIPAA-compliant systems outside of this website.
6. Changes to This Notice
We reserve the right to change this Notice and make the revised Notice effective for all protected health information we maintain. Updated Notices will be posted on our website and available upon request.
